Making documentation questions useful

A clear question helps a clinician understand what information is missing without steering the clinical answer.

Name the gap

Point to the part of the available record that is unclear or inconsistent, and explain why clarification matters for review.

Keep the clinical decision with the clinician

Ask for the clinician's supported assessment or clarification rather than suggesting an unsupported diagnosis or response.

Use patterns for education

When similar questions recur, review the pattern with your team. A short example or workflow adjustment may prevent repeated back-and-forth on future charts.

For general agency education. Review decisions should follow the patient's record and current applicable guidance.
