The reviewer compares available assessment responses with the supporting documentation. Start-of-care and recertification reviews may raise different questions, so share the relevant record for the assessment being reviewed.
Identify items that need clarification
A difference between an answer and the clinical narrative is a prompt to check the record, not a reason to assume a different answer. The responsible clinician should address questions according to your agency's process.
Return practical feedback
Our review flags inconsistencies and explains what needs attention. Agencies can request this as a standalone service or coordinate it with coding and plan of care review.
